If you're going to use a capture card, most of the times you'll need a PC too. Although I personally own an Avermedia Live Gamer Portable Plus 2, which has the option to record directly from console with a simple press of a button. It's the simplest form I know to record modern consoles. As far as I'm aware of, all you need is that your console comes with an USB port and HDMI ports too.
